Forms can be dropped or mailed to: Lemont Police Department 14600 127th Street, Lemont, IL 60439 Attn: Block Party Requests or emailed to: lemontpd@lemont.il.us Completed forms will be reviewed and applicant will be notified of approval. For your convenience, a Police Request Form is also available below.
